To establish a direct deposit of your pension payment sign into PSS. After logging in, select Direct Deposit from the Benefit Recipient menu.

This model order may be used in drafting a Qualified Domestic Relations Order for participants who are currently receiving pension benefit payments.

W-4P (Withholding Certificate for Periodic Pension or Annuity Payments)

Securely log into PSS to complete your IRS Form W-4P online.

You can also download a copy of the 2026 IRS Form W-4P or call the Benefit Funds Office to request to have a copy mailed to you. Once complete, you should return your paper IRS Form W-4P to the Benefit Funds Office for processing.

Physician’s Statement and Incapacitation Forms

Physician Statement of Incapacitated Benefit Recipient (PDF) 

This form and accompanying statements are used by agents submitting a Power of Attorney to act on behalf of a pensioner who is incapacitated due to a medical condition impairing their cognitive functions (e.g., dementia, Alzheimer’s).

Forms for Surviving Spouse of Deceased Pensioner under Spouse Pension Option

Forms for Surviving Spouse of Deceased Pensioner under Spouse Pension Option (PDF)

These forms are for surviving spouses who are entitled to benefits under a deceased Pensioner’s 50%, 75%, or 100% Spouse Pension. A death certificate is required to verify the death of a Pensioner.
